Elevation of creatine kinase, MB fraction after elective coronary intervention: a valid surrogate end point of poor late outcome?

چکیده

● Release of CK-MB is common if sought for in up to 25% of cases of percutaneous intervention; most commonly ,5 times the upper limit of normal. ● The causes may include angiographically evident events, such as branch vessel occlusion or embolization, but just as often they may be neither symptomatic or angiographically apparent. ● The likelihood of enzyme release is greater with nonballoon devices, such as rotablator, directional atherectomy and stents. ● Enzyme release is correlated with late mortality in some studies. ● The exact level of enzyme elevation to diagnose “myocardial infarction” with its attendant clinical and economic implications for patient management is uncertain, although an upper level of three times normal has been suggested (1).

بررسی اثر دوز بالای آتورواستاتین در 24 ساعت قبل از انجام آنژیوپلاستی کرونر انتخابی بر احتمال انفارکتوس حوالی آن

Background: Percutaneous coronary intervention (PCI) may been associated with high-er risk of cardiac events during this procedure. The goal of this study was to compare high dose atorvastatin therapy with low dose atorvastatin therapy 24 hours before PCI to a reduction in Peri- percutaneous coronary intervention myocardial infarction. Methods: One hundred ninety patients with stable angina were...

The sPLA2 Inhibition to Decrease Enzyme Release after Percutaneous Coronary Intervention (SPIDER-PCI) trial.

BACKGROUND Secretory phospholipase A(2) (sPLA(2)) may play a role in myonecrosis after elective percutaneous coronary intervention (PCI). Inhibition of this enzyme may have a beneficial effect. The central hypothesis of this study was that treatment with varespladib, a small-molecule inhibitor of sPLA(2) would reduce postprocedural release of cardiac biomarkers after elective percutaneous coron...

Minimal myocardial damage during coronary intervention is associated with impaired outcome.

AIMS Studies on the glycoprotein IIb-IIIa receptor blocker abciximab in patients undergoing percutaneous coronary intervention consistently show a reduction in procedure-related myocardial infarction. Some such infarcts are characterized by elevated creatine kinase or creatine kinase-MB, without apparent clinical symptoms. The clinical relevance of such 'creatine kinase leaks' has been question...

Long-term clinical events following creatine kinase--myocardial band isoenzyme elevation after successful coronary stenting.

OBJECTIVE We sought to evaluate the impact of intermediate creatine kinase-myocardial band isoenzyme (CK-MB) elevation on late clinical outcomes in patients undergoing successful stent implantation in native coronary arteries. BACKGROUND Elevations of CK-MB after percutaneous coronary interventions are frequent. An association between high level of CK-MB elevation (>5 times normal) and late m...

Creatine kinase-MB elevation after coronary intervention correlates with diffuse atherosclerosis, and low-to-medium level elevation has a benign clinical course: implications for early discharge after coronary intervention.

OBJECTIVES The study evaluated the incidence and predictors of creatine kinase-MB isoenzyme (CK-MB) elevation after successful coronary intervention using current devices, and assessed the influence on in-hospital course and midterm survival. BACKGROUND The CK-MB elevation after coronary intervention predominantly using balloon angioplasty correlates with late cardiac events of myocardial inf...
